Musikgarten Scholarship Program - Parent Application Form

Welcome to the Parent/Student Application form for the Musikgarten Scholarship Program. We are happy to support you and your child's musicial journey! Please fill out the application below. Click here if you prefer to fill out the PDF application.

By submitting this application the Family agrees to:

• Attend classes with both a child and adult. These are parent/child classes and both need to be in attendance.
• Participate in any parent orientation sessions the teacher schedules.
• Follow the teacher studios policies and guidelines.
• Support the child's musical activities at home by listening to the recording, singing and dancing.
• If for a Group Piano class, family agrees to provide an instrument, space, and time for practice recommended by the teacher.
• Assure regular attendance to classes and give reasonable advance notice if a class must be missed.

Consent to Participate

I give consent to participate in the Musikgarten Scholarship Program. I recognize that the teacher using the Musikgarten curricula is an independent business, sole proprietor, community music school, school, college or university, or music business and not an employee of Musikgarten/Music Matters, Inc. I understand that Musikgarten assumes no legal responsibility fo claims arising from this instruction, and agree to hold harmless Musikgarten from all legal claims arising in connection with such instruction.
